Lifespan was designed to assist clients in achieving their optimal level of health in the privacy of their own home. This is achieved by working with your physician and developing a specialized plan of care around your individual needs. Your plan of care is the carried out by our highly trained staff of healthcare professionals that include: Registered Nurses, Licensed Practical Nurses, Physical, Occupational, Speech, and Respiratory Therapists, Certified Home Health Aides, Social Services.

The members of our staff will arrange scheduled visits to you at home and can provide a variety of services dependent upon your plan of care. Services include: Skilled Nursing Visits, Physical Therapy, Speech Therapy, Occupational Therapy, Patient & Family Educations, Wound, Ostomy, & Catheter Care. Cardiac, COPD & Asthma Disease Management, Personal Care Services (bathing, dressing, grooming, etc.), Diabetic Care & Teaching, Medication Management, Blood Pressure Monitoring, Infusion Therapy, Anticoagulant Monitoring, Pain Management.
Medicare Part A covers 100% of the cost of necessary home health care. To qualify for Medicare to pay for Home Health Services 4 things must be present:
1. Your doctor must determine that you have a skilled need.
2. You must choose an Agency that is Medicare Certified.
3. You are confined in your home, leaving only for short period of time. During these short periods there should be evidence of effort and reliance on others to assist you as well as the use of assistive devices or special transportation.
4. Care must be intermittent (part time), medically necessary, and ordered/approved by your doctor.
Lifespan Home Health accepts new patients at any time. Referrals can be made by other healthcare professionals, community agencies, family members or neighbors.
